Welcome to the Division of Quality Assurance (DQA) Online Background Check Application.

This application is used to request an Entity Background Check for the following individuals:

  • Entity Operators: Persons applying for or renewing a license, certification, registration, or other form of approval from the Department to operate a care or treatment program (entity). This includes owners, legal representatives, and principal officers.
  • Board Members: Persons who are board members for an entity, provided they have client contact.
  • Non-Client Residents: Persons who reside in an entity facility, and who are neither clients nor employees.

NOTE: Entities completing employee, contractor, student, or volunteer background checks must use the Caregiver Background Check process.
