Welcome to the Division of Quality Assurance (DQA) Online Caregiver Background Check Application. This website is intended to be used for Entity Background Checks and should only be used when requesting caregiver background checks for the following individuals:

Entity Background Checks completed through this website are required to be kept on file with DQA. Results of these background checks are not returned to the applicants.

Individuals who are employees, contractors, students, volunteers, or any other person who does not fall into one of the three categories above should not use this application to request their caregiver background check. Instead, those caregiver background checks should be requested through the Wisconsin Department of Justice (DOJ) as outlined by the Employee Background Check process. Please submit those requests to DOJ either through a mailed in record check request form or through their record check website .
